Patients that have been admitted to a hospital for a minimum of three nights may have the option for a short-term stay in our center to receive 24-hour Nursing care and Physical, Occupational and Speech Therapy up to seven-days each week. Six days after his arrival at the nursing home he was sent to the local hospital and arrived oxygen deprived. Within hours he'd passed away. I am not sure what happened or the sort of care he'd received while at the nursing home, but after struggling to live for seven months he passed away within six days of arriving at Hunter's Creek Nursing Home. My father was in their facility for 21 days. He was discharged in WORSE condition than he was upon admittance to their “facility.” Not only did he receive no rehabilitation, but is now wheelchair bound (he was walking with a walker prior to being sent there from the hospital for rehabilitation) but his mental state has deteriorated severely. He has trouble communicating now. He is afraid to ask for things, afraid to ask us (his own family) for assistance getting up or going to the bathroom, he is afraid to ask for food or say he doesn’t like a food being served to him (this was not an issue before.) He sits in silence for hours at a time and apologizes for everything. He will ask a question and immediately say sorry for asking a question to us now. Upon picking him up he was distraught. He was leaving messages on my phone and other family members days prior just saying the words “help” “hurry” “help please” and “please get me out of here” he left voicemails accidentally of a CNA scolding him for not making it to the toilet. He left me a message on accident of him SOBBING in his room alone. Over one 4 minute voicemail I heard him BEG a nurse to help him out of bed and that he wanted to sit up and she LAUGHED at him and did not help him up. When I got the message I immediately called the nurse and she stated it sounded bad but he was “just up” and “he just wanted to get up again but he had just gotten up” He left us messages at 10pm, 11pm and later asking us to come to the facility to help him use the bathroom. (We have all of these messages saved) We questioned why he wouldn’t ask a nurse for assistance as we were not close by and we discovered why upon our next visit… his call button was deliberately pinned far away where he couldn’t ask for help. His personal items, snacks, grooming items etc were pushed far away where he could not access them. Our first visit we showed up and not only was he sitting in a wheelchair NAKED with just a gown on unattended IN A HALLWAY but his sheets were soiled and covered in BLOOD, and he couldn’t tell us why there was blood. We questioned the nurse and requested the sheets be changed. We cleaned him up and got him dressed and did his hair for him, put his things away appropriately where he could access them, and pinned his button to the bed so he could reach it but then we proceeded to wait for 1.5 hours for sheets so he could get back in bed and elevate his legs, but finally I went and found the sheets MYSELF in their linen closet and changed the sheets myself where I discovered they had PILED pads under his sheets like 8-10 pads thick so that he could just soil himself in the bed. I then discovered all of his underwear was MOVED in his dresser and replaced with DIAPERS …which he’s never used before because he was previously able to transfer himself to the toilet. Now he was afraid to do so or to ask for help getting out of bed to get to the bathroom. When I advised management (Marcus) about the blood and soiled sheets and how upset I was, I was word for word responded with “You're being dramatic.” from their patient services “team.” (Please note, nothing I am saying is without proof. I began documenting EVERYTHING the moment I got a bad feeling about this place because of the horrors we see about elderly abuse but had PRAYED this would not be the case with this place. So I do have that conversation recorded as well as all of the other conversations I’m going to mention, pictures of the blood and soiled sheets, as well as where his items were placed out of reach and his underwear being removed and replaced with diapers. Everything mentioned in this review I have documented so it cannot be he said she said situation.) **note**if you have a parent or loved one currently in this facility, please reach out to me via email as I may have documentation of neglect and abuse of your loved one as so much was documented inside this facility to protect the safety of my parent and myself as their team displayed SERIOUS retaliation against my parent after they were confronted and reported. This facility displays in their brochure and lobby how they have activities and a salon/barber services for their patients. This is not accurate. They have an activities room with no supervision and if a patient wants to go there, they must be able to make it to that room themselves (or pray they get a nice CNA that will take them IF they ask) as they do not ask or offer patients the opportunity to do anything other than sit in their bed/room quietly all day. Each visit I had to do my fathers hair for him as it was very overgrown and in disarray. On several documented occasions they let patients SCREAM at the top of their lungs “HELP..PLEASE HELP ME” and no one..not a single staff member went to check on them. My last visit before my parent was picked up, I confronted the “supervising nurse/floor leader” Vertrued about the screaming to which she responded “oh that’s behavioral. We don’t give her the attention she’s seeking” and then proceeded to ask other staff around her to back her up on that statement. I immediately said “that response terrifies me because lord knows how you treat my father when I’m not around.” To which one of the CNAs backing Vertrued up stated “oh no no..we would never” in the same breath they stated that about the screaming patient. When I stated that she could be comforted if it was behavioral instead of letting her scream in distress for the last 2 hours, she responded “well she stopped screaming now.” (She had for the first time in hours and lord knows why) I said how do you know this patient isn’t lying on the floor hurt ? She rolled her eyes at me. I then had to stop my response mid sentence because the hallway I confronted her in smelled like URINE so terribly I almost threw up. I pointed it out to which herself and the nurses she rounded up to back her up about the screaming patient proceeded to just blink at me when I asked if they were aware the whole area smelled overwhelmingly like urine. This is also documented. On day 21 my family received a call that my father was going to be sent to a shared housing facility in Orlando (this is basically a halfway house for the homeless) if my family didn’t come get him. I proceeded to tell the “manager of patient services” that I was EXTREMELY upset about this because no one had spoken to myself about this (to which he lied that he’d continually tried to reach me, I pulled my cell phone records and I was reached out to a total of TWO TIMES by the facility and once via text from this “managers” personal cell phone just 24 hours prior…anyone interested in seeing the cell phone records to prove this..I’d be happy to send all documentation over) he stated he spoke with my brother and he was aware (this is not an accurate statement and my brother immediately emailed the facility to call them out on this and stated he now preferred email so that all convos were documented since false information was being stated to which they did not respond) and when I responded I was not involved in that conversation he supposedly had with my brother his response (all documented) was “if you’re not involved then why are you calling me” When I asked if this shared housing had a nurse and was assisted living he proudly stated NO. It is just a shared home (with strangers) I got very upset and stated how could you do that to him he has dementia and is STILL completely wheelchair bound, he responded “my notes say he can transfer himself to the bathroom unassisted.” I said you are talking about dropping a wheelchair bound man with dementia who can easily be taken advantage of, can’t make rational decisions and could hurt himself very easily trying to simply feed himself or get up or use the bathroom and that was absolutely absurd and questioned why he was still wheel chair bound with no improvements…I was continually mocked. When I confronted this “gentlemen” in charge about why we were never sent a list of long term care facilities to research for him to get him into like they promised and why they did not help him file for Medicaid ICP as they promised, he responded with the lie that he tried to reach me. He then said what time can you come in tomorrow to fill out the Medicaid application. I said I’m an hour away and work, and the patient is in the facility with you now and has been for weeks, why do I need to be there to start this application with him that I was told would be done WEEKS ago, he responded with “well you said he can’t make rational decisions so I assumed you would want to be here to fill out the application for him or is he now able to make rational decisions. I’m confused” (again this is word for word and documented) at that point I got EXTREMELY upset and said this is ridiculous I am the last one to pull a “Karen” but we are talking about the safety and well-being of my father that I trusted in your care so if you can’t be professional then I want to speak to YOUR manager because I am trying to have a genuine conversation and you’re being very unprofessional, he then assured me he was the highest in charge and “your father would love to go home to your house BUT YOU DONT WANT HIM so we have to do something with him. He can’t stay here.” I said “are you serious ? I don’t want him? I live on a THIRD FLOOR APARTMENT WITH NO ELEVATOR ONLY STAIRS IN A ONE BEDROOM APARTMENT AND WORK 10 HOURS A DAY. HE CANT GET UP MY STAIRS AND COULD HARM HIMSELF WHILE IM NOT HOME WHICH WAS WHY WE WERE SUPPOSED TO GET A LIST OF LONG TERM FACILITIES WEEKS AGO AND GET HIM APPLIED FOR MEDICAID. I am barely an adult myself and have only my even younger sibling as family other than my father and we were assured by your facility that you would rehabilitate him, and help us get him into long term care while he was there, which you never did and now you’re threatening to drop him at a shared housing home for the homeless TOMORROW with no improvement to his condition, completely wheelchair bound and you have the audacity to say I don’t want him!!? I WANT HIM TO GET THE CARE HE NEEDS! If I could provide that environment to him myself don’t you think I would!!? I have been there every weekend visiting him and seen no improvement and I have tried to give y’all the benefit of the doubt and now you are throwing him out the door and making very offensive claims and you think this is acceptable?” He then HUNG UP ON ME. I called the facility back and asked to speak with a manager, I was then transferred to “Jose” I asked Jose what his title was because Marcus (the patient services manager) claimed he was the highest in charge but I did not wish to continue the conversation with him, Jose responded “if you were listening he’s the highest in charge in HIS department. I am the GM of the facility.” Even after the condescending response I tried to fill Jose in on what had transpired and how upset I was, (please note I advised and he consented to the conversation being recorded) half way through me updating him on what transpired JOSE HUNG UP ON ME. I called the facility back, got the front desk, advised I was speaking with Jose and I hope the call just got disconnected and I wasn’t hung up on by the GM , she placed me on hold and advised me “Jose says he will continue the conversation with you in the morning.” I responded with “I’m sorry? We were literally in the middle of a conversation…” and she replied “He said He’ll call you in the morning to continue. He’s walking out the door now.” I said “you are literally threatening to drop my father off in downtown Orlando IN THE MORNING. This isn’t something that can wait.” She stated “no one else is available to talk to you, I’m just the admin.” sat there in silence for a moment as I was in disbelief and then she ended the call. Jose in fact DID NOT call me to continue the conversation. The next conversation I had was my father texting me “help me. They’re kicking me out of my room and told me I’m leaving” I advised my father to not get on the bus or go anywhere and to have the facility call me immediately. He texted me back “Marcus said you have his number and you can call him.” I told my father to please ask the manager or nurse to call me. He responded they said they’ve called you all day (lie again…I have my cell records from my mobile provider. They did not call once) I reached out to a social worker from APS who advised me what they were doing was illegal and to call ACHA and file a complaint and advised me to forward her all this documentation to which I immediately did. During this call I got a phone call from Marcus (the manager of patient services) I asked ACHA to hold and I connected the calls together so they could hear what transpired. I advised Marcus that after his offensive statements yesterday and continuous mocking I would like to speak to someone else in his department (there’s two other ladies who work there that were in the office) and he laughed. I then stated I am on the phone with ACHA right now about this situation, so if you could have the other team member in your department call me in about 30 minutes to discuss, and before I could finish my sentence (with the ACHA rep on the line) he HUNG UP ON ME AGAIN. The ACHA rep gave me a bunch of resources to contact and as I was reaching out to those resources, I received a message from my father that they were packing up his belongings and asking him who was coming to get him if he was refusing to go to the shared housing home and that he owed them $350 a day for every day that he is there effective immediately. I immediately left work 2 hours away in fear of what would happen to my father and called ACHA back to update my complaint on what was transpiring. I was advised by multiple resources from them to call the local police department to meet me there. I called the facility and advised them of what I was told to do and that they better not transport my father anywhere before I got there. When I showed up my father was in the hallway with all of his belongings. The admin at the front was definitely aware of what transpired with Marcus and Jose as she assured me SEVERAL documented times that she was so sorry and understood why I was upset and could only imagine how I feel. I will not say her name because I fear they will retaliate against her too but our interactions were also documented. When she pointed me to where my father was sitting with his belongings in the hallway I confronted the nurses and advised them that I was about to call authorities and they then COMPLETELY tried to make it seem like nothing that took place ever happened and he was just “switching rooms”. The nurse tried to grab my fathers wheel chair and take him away from me and roll him into an abandoned room, this room had no sheets, no bedding, the bathroom was BARE with no toilet paper no paper towels and was the equivalent of a prison cell. I’ve actually seen nicer prison cells in my career. I documented everything that took place there. When I brought up if was just being room transferred to the room she wheeled him away to why was it completely bare and no bedding, no toilet paper or toiletries to even use the bathroom …they stated housekeeping was responsible and must have skipped the room. One nurse threatened me when she saw me recording and stated she was getting a supervisor which I gladly welcomed and this is when I met Vertrude who tried to approach my father like she knew him, patted him on the head like a freaking dog and tried to initiate a conversation in front of him, to which I immediately requested we step away as to not further upset or traumatize my father. Our entire interaction was documented and was extremely disturbing. This is another member of their staff that should NOT be allowed to work with patients or their family, after she continued to pretend like she had no information on his discharge or plan to be dropped off at shared housing, I immediately collected all of my fathers items off the floor in the hallway and I got my father safely in my car to leave. As I was leaving I was informed by my father that Marcus confiscated HIS WALLET and he did not have it. I went back inside to find out if this was true and normal practice (I had hid my fathers wallet in his luggage and only he knew where it was so I have no clue how they found it but my father assured me he didn’t give it to them but they in fact went through his belongings looking for his banking information and asking him for his payment information and how much money he collected from SSI and then took his wallet from him and told him he could just ask for it whenever he needed it) When I asked Vertrude about this I was met with eye rolls and she then advised me that no one was in facility that had access to get my fathers wallet back. I informed them I was not leaving without it. They advised Marcus was at home but he was on his way to “return it to me” and I proceeded to wait in their lobby for a total of 2.5 hours for Marcus to arrive. He then tried to hold my fathers wallet hostage unless I signed an AMA form. Which I would not do because I was not picking him up against medical advice, he was thrown out and was going to be discharged until they found out ACHA was now involved. I was finally able to get my fathers items but not before the social worker I kept on the phone for my safety witnessed Vertrude (sp?) proceed to talk terribly about me with Marcus VERY LOUDLY in their hallway because I asked the simple question “do you know how much longer this will take?” Long story NOT short….I have documented and reported all of this and anyone in fear that their family member may be one of the neglected patients I witnessed, please don’t hesitate to reach out. If you’re looking for a rehab for your loved one THIS IS NOT THE PLACE. This facility does NOT care for its patients and treat them terribly, traumatizes them, ignores them, scolds them, leaves them in their rooms for days on end. I witnessed and documented nurses bringing patients that were COMPLETELY incoherent lunch/dinner, sit it down for 30 minutes, and then come back and literally SCREAM the patients name in their faces, waving food in front of their face, scream “EATTTT” in their face and then laugh, pick up their tray and state “I left you a jello” slam a jello down with no spoon and leave the room with their food and the patient never ate a thing. This place should be shutdown and the staff there should NOT work in this career field..ever again. I hope my parents situation saves at least one person from picking their parent up from this hell hole in the traumatized condition my father is in. I will state there is a nurse there named Tammy, and my father credits her with single handedly keeping him alive and his spirits okay during his time there and that she did what she could to help him whenever she had a shift there. Her home health services were never set up afterward, no devices provided as they should have been, and even her discharge was disorganized. Trying to get her to understand and listen to the family member who is involved the most (ME) has been challenging to say the least. Dr. Soto is only at the facility 2 times a week and this frustrates me. In my father’s case because of the lengthy response times and the doctor absence, I have admitted my Father twice to the Emergency room. Had I waited any longer the infections would have over taken him. I am hoping we can resolve our communication issues moving forward. Dr. Soto is three star Physical therapy PT: Lilah the director of PT is such a great part of the facility. Lilah has only been there for a short period and is excited about helping your loved one. I have had the pleasure of working with Levonne & Emily PT techs on Lilah’s team. Emily is super respectful and knowledgeable of patients and their behaviors. Levonne has a more stern approach, which, sometimes, comes off as disrespectful. Since being removed from Hunters Creek, my loved one eventually made a 180 degree turn around for the better. If you truly want quality care for your loved one, THIS PLACE IS NOT IT. She told me even if she did punch the call button no one would come. They would put food in front of her but if she didn't eat they would just take it away. The rooms held either two residents or one; my grandmother stayed in a single-occupent room. The rooms seemed clean, but were very reminiscent of a hospital room.
